Are you asking how to build the firebox throat?
The throat is the part of the firebox that eventually becomes the chimney. Is this what you are talking about?

Put a piece of angle iron across and use the angle iron to support your firebricks, then put stones on top of your firebricks.

Just buy a chimney cap. You can probably find them at most hardware, home improvement stores.
Apart from that, I am sure you can find outdoor fireplace designs online.

Can't you but an insert to then cement the stones around it? Made of cast iron or fire clay? It may be the easy way out, but it's what I'd do.

Hire a mason who is experienced in building outdoor fireplaces. If you build it wrong it wont draw and you'll have to start over.
